We will help you get your license plate refund in a timely manner. You are required to destroy the license plate. You may bring it to us or turn it in to any MVD office. Personalized or Specialty plates do not have to be disposed of, but the month/year tab must be destroyed. If you were a lessee of the vehicle, your name must be recorded as lessee on the MVD vehicle record in order to have the refund issued in your name. Once you have requested a refund, the plate will be removed from the vehicle record and will not be valid until properly re-registered. There is a $12.00 processing fee required by Arizona law, which will be deducted from any refund you receive. A convenience charge also applies.

Law-enforcement officers will ask you for proof of insurance at the time of traffic stops or accidents. Insurance companies notify MVD of all policy cancellations, nonrenewals and new policies. If your insurance company sends MVD a notice that your policy is no longer active, MVD will send you an inquiry notice to verify insurance status.
Failure to maintain proper insurance could lead to the suspension of your vehicle registration and/or driver license. To reinstate these privileges, fees and future proof of financial responsibility must be filed with MVD. The future proof requirement is most commonly an SR22 form from an insurance company. This can be expensive to the vehicle owner, especially since the law requires the owner to carry the SR22 for three years from the date of suspension.
A suspension could have been generated for one of several reasons:
- MVD has not received proof of Arizona insurance.
- There was a lapse in insurance coverage.
- There was an accident case opened.
- A newly registered vehicle may not have been on system when insurance information was received.
- There was a VIN (vehicle identification number) error, either entered into the system incorrectly or provided incorrectly by the insurance company.
When you sell your vehicle or otherwise transfer ownership, or end your lease, you must notify MVD within 10 days. A Sold Notice is your protection from liability for tickets, accidents, etc. that occur after the sale.
Arizona law requires all driver license and identification card holders to report any change of address to MVD within 10 days. You will be asked to provide your name, date of birth and driver license or identification card number. There is no charge other than the usual convenience fee. If your current address is not on file, you may not receive renewal notices or other correspondence concerning your driver license or vehicle records. You may be cited by law enforcement if you do not change your address within the 10 days. When you change your address on our records, it will automatically change the address on your driver license record and on each vehicle record where you are designated to receive the registration.

Insurance Verification is a service that we offer to verify that the Motor Vehicle Division has received your most recent active insurance policy number.
The Motor Vehicle Division obtains this information from:
- Your insurance company through an electronic file (most preferred)
- You when you submit a copy of your insurance card
De-insurance is a method of temporarily not maintaining the required insurance on the vehicle until it is ready to be driven or placed on the road again. A de-insured vehicle will not be suspended due to lack of insurance unless it is driven on the roadways of this state. To de-insure your vehicle, stop by any one of our convenient locations so we can complete the De-Insured Certificate.
Level I inspections are for out-of-state vehicles without proof of title and/or registration, and for bonded titles. Vehicle inspections match the vehicle identification number (VIN) to the vehicle ownership documents to determine the identity of the vehicle, and (in some cases) to verify that the vehicle is properly equipped for highway use.
You will need three items for an inspection:
- Proof of ownership, usually a title
- Valid driver license
- Receipts or invoices for all component parts that have been repaired or replaced.

Plates may be personalized with a letter/number combination of your choice. Plates may only be issued to the owner or lessee of a currently registered vehicle. If leased, but not registered in your name, a document showing you as a lessee is required. Some plates may not be issued to large commercial vehicles (with a declared gross vehicle weight of 26,001 lbs or more). Some special plates may be issued to trailers, motorcycles and golf carts. Some plates are issued in pairs (front and back of the vehicle) and some are issued as a single plate, depending on the plate type. If your plate is personalized, two plates will be issued for full-size and one for small-size, regardless of the plate type.
A Medical Certification must be completed by an authorized physician, a registered nurse practitioner or by a hospital administrator. Applicants must have one or more of the following conditions:
- Unable to walk 200 feet without stopping to rest
- Unable to walk without help from another person or a brace, cane, crutch, wheelchair or other prosthetic or assistive device
- Lung disease with forced respiratory, expiratory volume for one second, if measured by spirometry, is less than one liter, or the arterial oxygen tensions is less than 60 mm/hg on room air at rest
- Uses portable oxygen
- Cardiac condition with Class 3 or 4 functional limitation as by American Heart Association standards
- Severely limited in ability to walk due to an arthritic, neurological or orthopedic condition
Many special plates offer the disability symbol option for both standard and personalized. On a personalized plate, the available number of characters is reduced by one to accommodate for the disability symbol.

Criteria (All three requirements must be met.):
- The vehicle is designed by the manufacturer primarily for travel over unimproved terrain.
- The vehicle has an unladen weight of eighteen hundred pounds or less.
- The vehicle has been issued an Arizona license plate.

When an Arizona title is lost, mutilated or becomes illegible, you, the owner of record, may apply for a duplicate title. Owner name, legal status, lienholder and vehicle information must remain the same. Only the address and/or vehicle location address may be changed. Photo identification may be required. The mutilated or illegible title must be surrendered at the time of application, when available; it is not necessary for the owner to sign-off the back of the title. When the record shows a lien, the duplicate title will be sent to the lienholder of record unless you present a lien clearance or power of attorney from the first lienholder of record and there are no other liens present on the vehicle record.
A Salvage Certificate of Title is issued to a vehicle that was stolen, wrecked, destroyed, water damaged or otherwise damaged to the extent that the owner, leasing company, financial institution or insurance company considers it to be uneconomical to repair.
Restored salvage is a vehicle that was previously issued a certificate of title branded as salvage, but has now been repaired/restored, then inspected for roadworthiness and issued a title branded as restored salvage.
We work with many Licensed Automotive Recyclers. Let us handle the paperwork while you concentrate on the dirty work! Before dismantling the vehicle, the recycler must have a current Arizona Salvage Certificate of Title. If you don't have one, we'll help you get it! The Arizona Salvage Certificate of Title is retained by the recycler and all lien clearance must be attached.
We help you complete the necessary paperwork to obtain a lien release from the lienholder/lender. All we need is from you is:
- A complete description of the vehicle (year, make, and vehicle identification number).
- The date the lien was paid in full.
- A notation or stamp that indicates the lien is paid.
- The full signature of the lienholder or the lienholder agent.
A Restricted Use 3-Day Permit allows a person to operate an unregistered vehicle or a vehicle with a suspended registration from the present location of the vehicle to a specified destination.
The Restricted Use 3-Day Permit is valid only for the following purposes only:
- Emissions Testing
- Vehicle Inspection
- Application for Title and/or Registration
- Vehicle repair to comply with Emissions or Inspection
The Restricted Use 3-Day Permit is valid only for these stated purposes. Travel for any other purpose, including commercial interstate movement, is strictly prohibited.
A 30-Day General Use Registration is issued when you do not qualify for a permanent registration or other types of temporary permits. Only one 30-Day permit is allowed per vehicle within any 12-month period.
A 90-Day Resident Registration is generally for individuals who are missing paperwork necessary to transfer an out-of-state vehicle title; two consecutive 90-Day permits may be issued within a 12-month period.
You can purchase a 90-Day Non-resident Registration. You will be required to show your out-of-state identification and the dealer invoice or assigned title.

There are eight reasons for requesting a duplicate Driver License or Identification Card. These reasons are lost, stolen, illegible, destroyed, extended expiration date to age 65, remove under 21, address change, or change the Driver License or Identification Card number when it is the social security number.
For alcohol- and/or drug-related revocations, you will be required to obtain proof of Future Financial Responsibility, called a certificate of insurance (known as an SR22). Bring the proof and your notice to any one of our convenient locations, fill out an application, pay the MVD reinstatement fee, addition to an application and Convenience fee. If you are required to have an ignition interlock device, call us before installation to determine if there are any actions on your record that would prohibit you from operating a motor vehicle. Proof of installation of a functioning interlock device must be provided prior to reinstatement.
